This Memorandum of Understanding (NtOU) is entered into by and between:
Community Works, Inc. (CW) and Central Point Police Department (CPPD).
Purpose The purpose of this MOU is to partner in providing a Community Works Co -located Victim Advocate
stationed at Central Point Police Department Community Works' mission is to strengthen lives and our
community through prevention, support services, and advocacy for victims of domestic violence, sexual assault,
and at -risk youth and their families Central Point Police Departments mission is first and foremost that each
and every member of the police department is a "Peace Officer," entrusted with the fundamental duty of
representing and defending the deeply held values of our community
Roles and Responsibilities:

A Reporting Requirements The victim advocate at Central Point Police Department will be responsible to
document the number of people served. types of services pros ided, demographic data, runnings
provided and received, outreach efforts, and submitting client feedback forms. Community Works
management will he responsible for submitting quarterly satisfaction surveys to CPPD. CPPD will be
responsible for tracking number of reports of domestic violence, sexual assault, stalking. and sex
trafficking
B Funding Community Works wdI bill Central Point Police Department monthly for staff salan and
perxmnel and associated project costs
C. Timeframe This MOU will commence on October], 2016 and wtl dissolve at the end of the grant
funding period on Sentember 30.2021.
D Confidentiality: In order to ensure the safety of clients, all parties to memorandum of understanding
agree to adhere to the confidentiality expectations required of them by statute and policy
This Memorandum of Understanding is the complete agreement between Community Works, Inc, and Central
Point Police Department and may be amended only by written agreement signed by each of the parties
involved
