HomeMy WebLinkAboutHazard Mitigation Grant ProgramSTATE OF OREGON OREGON EMERGENCY MANAGEMENT HAZARD MITIGATION GRANT PROGRAM CONTRACT DR-1632-OR CONTRACT FUNDING & PERFORMANCE PERIOD EXTENSION ADDENDUM No. 1 1.0 PARTIES TO THIS AGREEMENT This Agreement is made and entered into by and between the Oregon Military Department, Office of Emergency Management, hereinafter referred to as Grantee. and the City of Central Point hereinafter referred to as the ~Subgrantee• and is an addendum to the contract dated October 29, 2007. 2. PURPOSE Federal funding is provided by the Federal Emergency Management Agency (FEMA) and is administered by the Grantee. Under the authority of Presidential Major Disaster Declaration FEMA DR-1632-OR, the Grantee is reimbursing the subgrantee for those eligible project costs and activities necessary for the implementation of the Hazard Mitigation Project entitled Central Point Natural Hazards Mitigation Plan initially dated July 2007 and as described in subsequent application materials submitted to Grantee. 3.0 PERFORMANCE This contract addendum authorizes additional federal funding for project-related activities (Phase II) and extends the performance period of the subgrant through September 25, 2011. The modified Scope of Work and performance period extension for this project are approved by FEMA in their letter dated April 22, 2010. 4.0 FUNDING The amount of funding authorized by this contract addendum is $70,269 (Phase II) which is the 75% Federal share amount as approved by FEMA on August 17, 2010: The cumulative, total 75% Federal share amount for this project is $105,819 (Phases I and II). The parties understand that the subgrantee will contribute at least twenty-five percent (25%) of the total eligible costs for any eligible project activity. 6.0 PAYMENTS There are no changes to the procedures for the reimbursement of eligible subgrantee project costs as previously described in the contract.
