In coosidetatian for extending such insurance to its employes, the Public Agency submits for the Board's approval this Agreement
to become effective on the date hereinafter sated, and promises and agrees to be bound by the terms and conditions herein expressed.
ARTICLE I.
Definition(
For the purpose of this agreement:
(a) The term"employe"means an employe of the Public Agency rendering to it services as herein defined and limited.
(b) The term"services"means all services performed by individual as employes of the Public Agency,except:
(1) Any service performed by an employe in a position coveted by a retirement system the date this agreement is made
applicablemitts coverage group.
(2) Service performed by an employe who is employed to relieve him from
(3) Service performed in a hospital,home or other institution by an inmate
(4) Service performed by employes of transportation systems which are under Federal Old-Age and Sur-
vivoa Program or some other program.

(6) Part-time services for which the compensation is on a fee basis.

(9) Emergency employment not in a regular position and sixty days in a calendar year.
ARTICLE IL
Services
(a) This agreement include any services in regular fu • time positions listed is the employer's budget estimate, or
similar positions officially established after the estimate is made • designated as usual items of expense,excepting those -
riots excluded in Article 1,section b,sub-sections 1 to 9,i • above, and excepting items excluded in section b of this

(c) Within 20 days after the final pay-paiod in each quarter,the Public Agency shall furnish the Board,on forms prescribed by
die Social Security Administration,an oiminal and one copy of the Quarterly Report, listing the name and Social Security number of
each person who has been on its payroll during the quarter, the gross amount of compensation earned by,sod the total Social Security
deduction withheld from,each of the employes during the quartet
(d) At the end of each calendar year the Public Agency shall furnish to each of its covered employes a statement showing total
grass compensation tamed by him during the year and the total Social Security deductions withheld therefrom during the year.
(e) The Public Agency shall furnish every terminating employe a statement showing his total gross compensation earned in the
calendar year in which he terminated and the total Social Security deduction withheld therefrom during that same period.
(f) The Public Agency shall follow the instructions furnished to it,by: requiring cads dye to establish a Social Security ac
count number: reporting the accounts of employes who have applied for but have nor yet received Social Security account numbers:
verifying account numbers for employes who have already established accounts: completing quarterly among and complying with any
other directive, rule,or regulation of the Social Security Administration or the Board,relatirep

erage.

Coasdbaaionr
' (a) The Public Agency shall be liable for the contributions required of an employer by section 1410 of the Federal Internal Rev-
enge Code and amendments thereto,except that no contributions shall be remitted prior to approval of the Sate-Federal agreement by
the Federal Security Maims— imam
(b) The Public Agency shall withhold from don paid by it to officers and employes covered by this agreement,and re-
mit to the Board,that pauon required to be withheld tom the salaries and wages of employes under the provisions of section 1400
of she Federal Internal Revenue Cade and amendments thereto.
(e) 'The Public Agency shall reimburse the Board for is prorate share of expenses incurred in the administration of Chapter 454,
Oregon Laws 1951,and the execution of functions required by the Federal-Stare agreement,made in conformance with said kw. Suck
reimbursement shall be made quarterly and shall be based on the number of covered employes on the payroll of the Public A .- ty dur-
ing the quarter.In computing die expense of administadon,the Board shhall,during each of the first five years following the elective
date of the first agreement executed under Chapter 454,Oregon Laws 1951,include as expenses of administration the sum of$250.00
per quarter in excess of the actual expenses incurred.
ARTICLE V.
Adjariaaatr,Re/x.&and Penalties •
(a) If mote or less than the correct amount of any payment due under part IV of this • is remitted,proper adjustments,

(a) The Public Agency may request the Board to give notice the Sec city Administrator,pursuant to section 218 (g)

the Social Security Act and the laws of Oregon.
